Our values include empowerment, empathy, compassion and innovation. PROGRAM/POSITION OVERVIEW Woodville Alliance (WA) delivers a range of integrated programs and services for adults with intellectual and developmental disabilities. The programs and services are either centre based or non centre based and include Community Participation and Transition to work. The WA Disability Services Program utilises a positive strength-based philosophy that seeks to engage clients with the community to maximise client outcomes. The Disability Services Program practice is guided by the NDIS Practice Standards.The Support Coordinator is responsible for supporting NDIS participants in achieving their individual goals through coordination of support services. The position will support the development and implementation of NDIS participant plans, so as to maximise positive outcomes and achievement of individual goals for each client. The role strengthens outcomes for both the client and families through integrated and coordinated service delivery. MAIN DUTIESBuild and maintain pro-active relationships with NDIS registered non-registered providers. Monitor plan outcomes and expenditure. Take a holistic approach by working effectively with the participant to coordinate, implement and maximise on identified NDIS plan goals. Deliver clear accurate and timely record keeping for compliance purposes. Ensure timely and accurate billing. Ensure client hours are billed at 75% and above. Growth of support coordination.  ESSENTIALTertiary qualifications in Social Work, Psychology or related field, or experience deemed to be the equivalent.Strong experience in the development and delivery of sustainable outcomes for people with an intellectual and developmental disability as well including from a CALD background. Knowledge and understanding of disability sector funding including NDIS.Ability to engage and manage complex issues related to servicing Culturally and Linguistically Diverse clients.Demonstrated superior skills in mediation and negotiation, collaboration and consultation with key stakeholders.High-level knowledge and understanding of the policy, procedures and standards pertaining to the provision of disability services. Commitment to social justice principles and access and equity.Demonstrated high level customer/sales and networking/marketing service skills. Willingness to learn and drive the growth of support coordination at Woodville.  HOW TO APPLYPlease click the 'apply' button to forward your CV and Cover letter addressing why you would be a good fit for this role.
