Disability Support Worker – Permanent Part Time (55 hours per fortnight)  POSITION OBJECTIVEWA delivers a range of integrated programs and services for adults with intellectual and developmental disabilities. The programs and services are either centre based or non-centre based and include Community Participation, Transition to Work, Leisure Link, Life Choices, Self-Managed and Supported Living Services. The position is responsible for working day-to-day with clients in understanding and assisting them to achieve their goals. Disability Support Workers are to assist the clients, by adhering to NDIS plans that are designed to maximize positive outcomes and achievement of individual goals for each person.     MAIN DUTIESWork with clients on a day to day basis to encourage, motivate them to meet their goals, recording their progress and evaluating and identifying ongoing steps to achieve their goalsEnsure that person centred practice and a culture of choice is upheld, promoting values of dignity, respect, privacy and integrityUphold the NDIS and WA Code of Conduct and adhere to NDIS Practice StandardsParticipate in transport runs, transporting clients to and from the centre or to community participation activitiesAssist clients in support activities including feeding and personal careContinuously encourage, motivate, acknowledge and respect all Disability team members to ensure quality service is being providedMaintain positive working relationships with families, colleagues and stakeholders, providing a high standard of customer serviceStrict adherence to the roster, ensuring availability during award hours (6:00 am – 8:00 pm, weekdays and weekends)Commitment to your Duty of Care/WHS requirements and WA policy and proceduresParticipation in client activities including hydrotherapy          ESSENTIALMinimum Certificate III in Disability Work or equivalentAt least 1 year in a care capacity of people with an intellectual and/or developmental disabilityValid Drivers’ License and willingness to do transport runsWillingness to provide personal care to both male and female clientsAvailability during award hours (6:00 am to 8:00pm, weekdays and weekends)Knowledge of NDIS Practice Standards and NDIS Code of ConductHigh standard customer service skills HOW TO APPLYPlease visit http://careers.woodville.org.au/ and click "Apply" to forward your CV and letter of application addressing the essential criteria 1 to 4 listed above by COB Monday 15th July.
